Community solutions and system transformation

At the Community Health Commission of Missouri (CHCM), we believe that addressing persistent racial and communal disparities requires a dual approach: community-led solutions and systems transformation.

These two forces work in tandem, reinforcing each other to create lasting systemic change and healing for our communities. Our vision is to achieve zero health disparities, a goal embedded in our theory of change and carried out through strategic community-system partnerships and collaboration.

What is EC-TIC?

Equity-Centered, Trauma-Informed Care (EC-TIC) is more than a guiding principle—it is the foundation that weaves all of our work together. Every initiative at CHCM, from training and technical assistance to policy implementation, is shaped by an understanding of the profound and lasting effects of trauma and structural inequities.

By prioritizing healing, agency, and resilience, EC-TIC ensures that our solutions are not only equitable but also sustainable. From reimagining how resources are distributed to transforming who leads the research and decision-making processes, our approach ensures that equity is not an afterthought, but the foundation of all that we do.

Partnerships & Collaboration

We believe that sustainable, transformative change must be community-led. CHCM builds partnerships that center the expertise, leadership, and priorities of those most impacted by health inequities. We work in deep collaboration with grassroots organizations, neighborhood leaders, and community-based advocates to ensure that solutions are driven by the needs and visions of the people they serve. By bringing institutions, policymakers, and funders into this community-centered framework, we strengthen the collective impact of our initiatives and ensure that solutions remain responsive, sustainable, and rooted in equity.

Equity-Centered, Trauma-Informed Care

At CHCM, all of our work is grounded in equity-centered, trauma-informed care. We recognize that health disparities are deeply intertwined with systemic injustices and historical trauma. Storytelling and accountability are fundamental to our work. Participatory Budgeting

Participatory budgeting empowers community members to have a direct role in deciding how funds are allocated to create stronger communities. Our Seniors Community Fund is a pilot project allowing community members to shape funding priorities and solutions to address their specific needs.

We believe that research should be by and for the community. Through our Community Institutional Review Board (IRB), we ensure that research conducted in our communities is owned by community members themselves, ethical, culturally competent, and directly benefits those involved.

Our Theory of Change

Strategic community and system partnerships help us advance our mission of eliminating health disparities in Missouri. Through tools such as participatory budgeting, participatory research, policy-in-practice, and pipeline for workforce developed—supported by storytelling and system accountability—we drive change with an equity-centered, trauma-informed lens. Together, these efforts allow us to meaningfully reduce and ultimately eliminate health disparities across the state.
