City of St. Louis Department of Health: Health Equity Plan

Partnering to Build a Healthier, More Just St. Louis

The City of St. Louis Department of Health (DOH) partnered with the Community Health Commission of Missouri (CHCM) to develop and implement a refreshed health equity plan – a strategic initiative designed to address systemic health disparities and improve health outcomes for all city residents .  

Through this collaboration, CHCM played a lead role in ensuring that the plan reflects both data-driven insight and community experience, grounding the city’s public health strategy in equity, engagement, and evidence. 

Our Role in the Health Equity Plan

  • Lead Partner in Plan Development: We supported DOH in researching, designing, and drafting the updated Health Equity Plan—ensuring it is rooted in community priorities and aligned with best practices for advancing health equity.
  • Data-Driven Approach: Our team conducted in-depth public health data analysis, engaging historically marginalized communities to ensure their voices and lived experiences shaped the city’s equity goals.
  • Ongoing Implementation Support: We remain actively involved in putting the plan into action—partnering with DOH to translate insights into policies, programs, and partnerships that drive real, sustainable change.

What’s Next

Although the Health Equity Plan has not yet been publicly released, our collaboration with DOH continues. We are committed to ensuring that community voice remains central to every step of implementation—and that this work contributes to a healthier, more equitable future for all St. Louisans.
