Welcome to the new digital home of the Community Health Commission of Missouri.

We’re proud to reintroduce ourselves as a newly relaunched, equity-centered, trauma-informed public health intermediary. We work across systems, sectors, and communities to achieve one bold goal: zero health disparities.

While our name may be new, our legacy is rooted in decades of community trust, policy advocacy, and systems transformation across Missouri. CHCM was born from the merger of Alive and Well Communities and the St. Louis Regional Health Commission—two organizations committed to changing the conditions that produce poor health outcomes and racial injustice. Together, we carry forward a shared vision and renewed strategy to move upstream, shift power, and center communities in the work of health equity.

At CHCM, we believe that health is not just an outcome—it’s a right. And achieving health equity requires more than programs and services; it demands partnership, policy change, and the courage to reimagine what’s possible.
