In partnership with the City of St. Louis Department of Health (DOH), CHCM supported the development of a refreshed health equity plan focused on eliminating systemic health disparities and improving health outcomes for all residents.
Our team led data collection and analysis, facilitated community engagement and designed a strategic framework rooted in the lived experiences and needs of historically marginalized communities.
Beyond plan development, we continue to partner with DOH to implement key strategies, ensuring that data-driven insights lead to meaningful, community-centered change
Community Engagement & Listening Sessions
As part of our shared commitment to equity-focused community engagement, CHCM and DOH are co-hosting a series of community listening sessions in partnership with Health Stop, focused on sexual health and other critical equity issues. These sessions provide space for residents to share their experiences, elevate concerns, and help shape public health priorities that reflect the realities of diverse St. Louis communities.

Leveraging Insights for Statewide Impact
This partnership with DOH not only benefits St. Louis—it also strengthens CHCM’s statewide equity agenda. By working closely with local leaders and community members, we gather insights that inform our broader work in health equity, racial justice, and systems-level policy change across Missouri.
