Partnering to Advance Community-Led Healing Across Missouri

Missouri Foundation for Health

At the Community Health Commission of Missouri (CHCM), we are committed to eliminating health disparities through community-driven solutions and equity-centered, trauma-informed care.

With the partnership and support of the Missouri Foundation for Health (MFH), we are expanding initiatives that promote mental well-being, address trauma, and prevent violence—rooted in the lived experiences of communities across the state.

Community-Led Solutions in Southeast Missouri

This initiative focuses on locally led healing efforts that build mental well-being, resilience, and equity in Southeast Missouri. By centering the voices and leadership of those most impacted by trauma and adversity, we are helping communities access the tools, resources, and support they need to drive meaningful, long-term change.

Grounded in collaboration, trust, and shared leadership, our work recognizes healing as more than an outcome—it’s a collective process. Together with MFH, we are investing in a future where communities are not only heard, but empowered to lead.

Building a Blueprint for Healing

This initiative is more than a project- it’s a movement to embed healing-centered approaches into the fabric of communities. By uplifting local voices, fostering safe spaces, and promoting culturally responsive mental health strategies, we are creating a model that can be scaled across Missouri and beyond.
